At the Monterey County Sheriff's Office, we offer a variety of forms to help you report incidents and submit official requests. Each form is designed for a specific type of report or service, such as criminal history checks, employee complaints, fraud, or ride-alongs. The sections below contain detailed descriptions and examples to help you choose the correct form. Enforcement Incident Reports & Requests
Incident reports should only be submitted if the crime occurred within the unincorporated areas of Monterey County. Crimes that occurred within jurisdiction of cities should be reported to the respective police agency.
If you’re unsure which jurisdiction an incident occurred in, Monterey County’s Emergency Dispatch offers a service which is useful in determining which agency will have jurisdiction over the incident you’re reporting. Please click here to access the Spark Online Reporting service. Once you’ve entered an address, a pop-up will occur if the reporting service recognizes the location’s police jurisdiction.

Description: You have information about a crime and would like to provide it to our department. This information can remain anonymous if you would like. If you do leave contact information, our staff may reach out to clarify vague details. If this is about a crime that is occurring or has just occurred please stop and call 911.
Example: Submit this form if you want to report overhearing people talking about a crime that they committed several days earlier. More generally, this form is to report crimes that have already occurred, and are not actively occurring. Again, if a crime is actively occurring, please stop and call 911.

Description: Members of the public can request arrest records maintained and originated by the Monterey County Sheriff's Office. You will receive a letter from our office; if no records is found, this will clearly be indicated, and if any records are found, they will be compiled into the letter. The letter our office compiles will include any booking and arrest information, any charges, and any warrants issued against you.
Example: Members of the public submit this request for several reasons. For immigration purposes, trying to clear their arrest records with lawyers. For moving internationally, starting a new job or moving to a different country.

Description: Members of the public may submit a complaint form regarding their experience with any Sheriff's Office employee. Complaints may include unprofessional behavior, misconduct, excessive use of force, or other concerns related to the employee's conduct. All submissions are reviewed by internal staff, and, when appropriate, a formal investigation will be conducted. Depending on the outcome, corrective action or disciplinary measures may be taken.
Example: Submit this form if you experienced a disrespectful or unprofessional interaction with a deputy or staff member, such as being treated unfairly during an interaction or receiving poor customer service at a Sheriff's Office administration building. You may also submit a complaint if you have constructive criticism about how an employee acted during an arrest, investigation, or public encounter.

Description: If you would like to report a crime that does not fall under a specific category such as fraud, vandalism, littering, or lost/stolen property, please use this General Crime Report form. This form allows you to describe incidents that may not have a designated report type but still require documentation or follow-up by the Sheriff’s Office. All submissions are reviewed by staff and may be referred to a deputy for investigation, depending on the nature of the incident.

Description: Members of the public can submit a report detailing an incident where their identity was used by another individual and any other victim, witness, and narrative information about the fraud incident. Another person obtains a credit, purchases items or services by using your Personally Identifiable Information (PII), like your social security number, drivers license number, credit card numbers, etc.
Example: Submit this form if someone opens a credit card account or obtains phone service using your personal information. An alternative scenario would be someone using your credit or debit card to make purchases.
Please note, if you are the victim of identity theft or fraud, keep all documents, receipts, cancelled checks, mail correspondence, and any other paperwork or evidence associated to the incident. You may be contacted at a later date by an investigator if these items are needed.

Description: Members of the public or other parties can request arrest records or crime/incident reports, if they have the authority to receive the information. Our staff will only search for records originating and maintained by the Sheriff’s Office. Arrest records will include booking and arrest information, any charges, and any warrants issued for the requested person. Incident reports will include the narratives from the arresting officer.
Example: You may submit this request if you’re an attorney requesting a client’s arrest report for court proceedings, or if you’re a member of the public seeking a copy of an incident report related to a theft or dispute you were involved in. Additionally, welfare or social service agencies, may request you submit arrest records for verification or investigations.

Description: PC602(O) allows law enforcement, at request of landowner or valid agent, to lawfully enforce trespassing laws on a specified property. After submitting this request, Sheriff’s Deputies will have the right to arrest trespassers without issuing warnings. This request only lasts for a 30 day window.
Example: Submit this form if you own a large property somewhere in Monterey County and want Sheriff’s Deputies to enforce trespassing violations.

Submissions
Forms can be submitted in-person or via mail to any of our stations, we have three stations throughout Monterey County.
